How to Request Your Health Records
Follow these simple steps to submit your request securely:
Step 1: Start Your Request

- You will be required to enter your email address.
- You will receive a secure link via email to access the request manager.
- A username and password is not required.
Step 3: Submit Your Request

- Healthmark will process your request.
Step 4: Receive Updates
- You will receive regular emails with instructions until your record is available to download.
Processing and Timing
Please expect processing delays and updates via email once your request has been submitted.
- Standard Medical Record Requests - Generally take about 2 to 3 business days for processing.
- FMLA and/or Disability Forms - Take an average of 7 business days. These forms require more detailed information and approvals from the physician.
